New AVC investment Choices
(active members)

 Should you wish to change your investment choices for your future Additional Voluntary contributions and/or accrued AVC funds you will need to complete and return this form.

(deferred members)		 Should you wish to change your investment choices for your accrued AVC funds, you will need to complete and return this form.
   
Expression of Wish

This form lets the Trustees know to whom you would like any lump sum death benefits to be paid.

Each time your family/personal circumstances change (eg on marriage/change of partners, divorce, birth of a child etc), you will need to give careful consideration as to whether or not you need to change your Expression of Wish.
